SECTION 5: INDEPENDENT CONTRACTOR STATUS ACKNOWLEDGMENT

Do you acknowledge and agree to the following statement regarding independent contractor status? By selecting “Yes,” I acknowledge that I am applying for an independent contractor (1099) role and not a W-2 employee position. If selected to provide services, I understand that I will be responsible for my own federal, state, and local taxes and for maintaining my own insurance coverage, certifications, and required credentials. I understand that I am not eligible for employee benefits, workers’ compensation coverage, unemployment insurance, paid leave, or health insurance through NOW Soccer Academy. I understand that any contracted services will be governed by a separate written Independent Contractor Agreement and that nothing in this application creates an employment relationship or guarantees a contract. *

SECTION 6: CONDUCT & PROFESSIONAL STANDARDS ACKNOWLEDGMENT

By selecting “Yes,” I acknowledge that, if selected to provide contracted services, I will comply with all NOW Soccer Academy policies, child safety standards, league rules, and codes of conduct. I agree to maintain professional boundaries with players and families, including appropriate in-person and electronic communication. I agree not to use Academy contact information, branding, curriculum, training methods, or proprietary materials for personal or outside business purposes without written authorization. I understand that failure to comply with these standards may result in termination of any contract relationship. *

SECTION 7: NON-SOLICITATION & LIMITED NON-COMPETE ACKNOWLEDGMENT

Do you acknowledge and agree to the following restrictions regarding solicitation and competition? By selecting “Yes,” I acknowledge that, if selected to provide contracted services, I may be required to sign a separate Non-Solicitation and Limited Non-Compete Agreement. I understand that such agreement may restrict me from directly or indirectly soliciting Academy players, families, or staff for competing organizations or private training outside the Academy during the term of my contract and for a limited period following termination of the contract. I understand that violation of such agreement may result in termination of the contract relationship and potential legal action where permitted by law. *
